**NOTE: Due to strict safety and liability reasons of Therasage Canine and host facilities, if you are pregnant at the time of the selected class you will be required to attend a class at a later date.
This is due to safety and liability reasons that Therasage K9 or the host facility will not be responsible for.
TUITION & PAYMENT
The total cost of tuition is $1,850.00, with a $300.00 registration fee (a $18.00 surcharge will be applied to the charge) due at the time of enrollment, which is non-refundable if the student cancels or is a no-show. A balance of $1,550.00 is due on the first day of class. The cost includes manuals, handouts, and training in all modalities KT Taping, PEMF (MagnaWave), Red Light, and more. Due to the educational aspect and proprietary information associated with Therasage K9 class materials and instruction, there is NO REFUND for anyone who has attended any class or portion of the class. 100% attendance is required for a certificate to be issued.
SIGNED CONSENT
I certify that I am in good health and fully capable of participating in the Therasage K9
Certification Program and that the information I have provided in this application is accurate and factual. If there are any uncertainties about the participant’s canine handling experience, Therasage K9 reserves the right to request the individual to leave and learn the necessary handling skills before returning to complete the certification. Proper handling would include being comfortable working with any size or breed of dog. Engaging in canine massage and handling carries inherent risks. Therasage K9 takes precautions to ensure safety, yet unforeseen reactions or responses from the dog may occur. I acknowledge that dogs are unpredictable animals that can cause damage, injury, or even death and that in working on or around dogs, I am accepting
this risk and agree to hold harmless Greg Gage, Therasage K9, Therasage EMC, any
instructor, and any kennel, kennel owner, dog owner, or anyone else associated with this program.
